Our Blood Pressure Self-Monitoring (BPSM) program is a research-based program designed to help adults with high blood pressure develop the skills and confidence to manage their condition.

Through self-monitoring, education and support, participants learn how to track blood pressure, understand numbers and make sustainable lifestyle changes.

This program focuses on prevention and long-term health, supporting participants in building daily habits that improve heart health and overall well-being.
