Support My Patients


RecoveryRx is a path to connect your patients to our YMCA offerings to complement their plan of care. Our programs can support your patients in managing diabetes, hypertension, overweight and more.  Refer your patients to the Y via our HIPAA-compliant referral portal at or securely fax at 1.888.965.7560.

What the Y Does

The patient will receive a call to determine which program supports their disease prevention or management.*  Then, they will receive sessions with Y staff and access to YMCA facilities FREE for a limited time at selected locations.**

*Programs may be fee-based.
**Patients limited to one limited-time free access.  Referrals must be made directly to the YMCA from the health care provider.  Walk-in patient referrals are not eligible.

How the Y Follows Up

The YMCA will send you semi-annual, aggregated reports of your office or department referrals.

Diabetes Control

Looking to lower you HbA1c? The YMCA’s Diabetes Control Program is designed for adults (18 years and older) living with type2 diabetes in need of tools and support to manage their condition.  Class is group discussion-based and are delivered over 12 weekly, 90minute sessions.  To qualify for the program, participants must have been diagnosed or have an HbA1c blood value of 6.5% or higher.  Prevent complications and enhance your quality of life by taking control of your diabetes! 


El programas de Control de Diabetes de la YMCA está diseñados para ayudar a adultos (18 años en adelante) que viven con diabetes tipo 2 a que necesiten herramientas y apoyo para manejar esta condición.  La clase es basada en discusión en grupo y facilitada a lo largo de 12 sesiones semanales de 1.5 horas de duración. Para calificar al programa, los participantes deben haber sido diagnosticados o tener un valor de HbA1c de 6.5% o más.
