REGISTRATION FORMS
The completion of all forms is required by Virginia Department of Social Services licensing regulations. Please return them two weeks prior to your child starting camp.  Your child is not registered until your Shady Grove Family YMCA Registration Packet and appropriate payment contract have been returned to the Shady Grove Family YMCA. Your YMCA membership and account must remain in good standing for the duration of the program.
